New Patient

Q:

Does my insurance cover my visit?

A:

We currently contract with Medicare part B, Blue Shield, 1st Health, Interplan, and Private Healthcare systems.  Many insurance companies offer out of network benefits.  Please call your insurance company to verify your benefits. We can provide you with a billing statement that you can submit to your insurance company for reimbursement. Kaiser and HMO’s will not cover our services.  We do accept cash paying patients as well.

Q:

Do I need a referral?

A:

Yes, we require a referral from either a Neurologist or Primary Care Physician in order to become a patient here.

Q:

How long are the appointments?

A:

The initial appointment is usually two hours long.   This includes check in as well as your visit with the Doctor. The follow-up appointments are scheduled based on patient need and average 30 minutes.

Q:

What do I bring to my appointment?

A:

Please bring your insurance cards, new patient packet, and any additional medical records you have that haven’t been forwarded to our office.

Q:

Are there any special preparations I need to make for my appointment?

A:

No, make no changes to your daily routine.

Q:

Will I see the same Doctor each time?

A:

Yes, you will be established with your Doctor, and see only that Doctor, thus providing you with consistent care.  In the event (illness, emergency) that your Doctor cannot see you on your appointment day, you will have the choice to see another Doctor, or reschedule.

Q:

After my appointment, what do I do if I am not feeling well, or I am having problems with medication adjustments?

A:

We have an advice Nurse on staff Monday – Friday 8:30 – 5:30 that will help with any issues that arise.  Call 408-734-2800 and ask to speak to the advice nurse!

Q:

Will you be my main Neurologist, or do I continue with my current Neurologist?

A:

Everyone is different.  Some patients only see us for Neurological care, while some check in with us once or twice a year while they continue to see their local Neurologist.  The choice is yours!
