You can review, download, print, and fill out the New Patient Information Packet before you arrive to save time and make your first visit go more smoothly. Please bring the completed forms with you on your first visit to the clinic or FAX to (408) 734-9208 before your visit.

NEW PATIENT PACKAGE

Initial Appointment

The Parkinson’s Institute focuses on specialty care. The following events take place on a patient’s initial visit with a neurologist at the Parkinson’s Institute:

  • Vital signs taken
  • Detailed review of past medical history
  • Neurological assessment
  • Movement assessment
  • Diagnosis
  • Summary of appointment
  • Treatment plan/next steps

The initial appointment lasts approximately one hour.

Follow up Appointment

The Parkinson’s Institute focuses on specialty care. The following events take place during a patient’s follow-up visit with a neurologist at the Parkinson’s Institute:

  • Vital signs are recorded
  • Neurological assessment
  • Movement assessment
  • Medications review
  • Summary of appointment
  • Treatment plan/next steps

A follow-up appointment lasts approximately 30 minutes
